Sometimes, though the individual carriers writing HIV generally use full underwriting. Group cover through an employer needs no individual underwriting at all, and guaranteed issue asks no health questions.
No-exam and HIV are compatible, but the best route is often not the one people look at first.
Group cover is the overlooked answer
Employer group life is issued without individual medical underwriting up to the guaranteed issue amount. No exam, no questions, no test. For many HIV-positive employees this is the cleanest cover available, and it is frequently already sitting there unused.
The thing to understand before you need it is the conversion right — group cover ends when the job does, and converting it later requires no medical underwriting if you act inside the window. See group conversion.
The individual no-exam products
Accelerated underwriting reads prescription data, so antiretrovirals will surface and the file will generally be routed to full underwriting. That is a delay rather than a decline, but the speed advantage is gone.
Simplified issue commonly includes HIV among its knockout questions, and carriers vary on this.
Guaranteed issue asks nothing at all. Small face amounts, graded death benefit, but a genuine floor.
The realistic sequence
Check what group cover you already have. Then approach the individual market through someone who knows which carriers currently write HIV — expecting a fully underwritten process, and having three years of viral load and CD4 history ready.
